  • Refers to programs related to health education, nutrition, transportation, and income support
  • Screens for Medicaid nursing facility and home and community-based service waiver programs
  • Offers case management for older adults and consultations and seminars for caregivers- Assists with applications for the Homeowners' and Renters' Tax Credit program
