Eligibility
Residents of Montgomery County who fall under at least one of the following categories: Teen mothers; individuals with no prenatal care; those experiencing substance use or misuse; mental or behavioral health concerns; intimate partner violence; unstable housing or homelessness; a history of previous infant death; a history of previous preterm birth; very low birthweight infants (less than 1,500 grams); and any other circumstance deemed to pose a serious risk to the mother or infant.
